Enrolment Application Form

With the Enrolment Application, the following documentation is required:

• Birth Certificate
• Baptism Certificate (if baptised Catholic)
• Immunisation Certificate (up to date and current)
• Any relevant documentation from the kindergarten
• For students not beginning Foundation and already at school; latest school report
• Reports from specialists who have seen your child
• Visa / Citizenship / Passport records if child not born in Australia
• Visa / Citizenship / Passport records of any parents not born in Australia
• Proof of residency (rates, utilities bill).
